COVID INFORMATION

Covid-19 Safety Guidelines for Our Staff and Clients.

To protect the safety of our staff and clients, we are asking that all clients receiving in-person services in our office to please wear a mask even if fully vaccinated. If you enter the office without a mask, one will be provided to you. Hand Sanitizer will be made available in our office, please feel free to use as needed.

Please note, the following face coverings are not allowed in our office:

bandanas, scarves, and masks with exhalation valves, However, if you have a documented medical reason prohibiting you from wearing a medical mask, please let us know. 

Complete your COVID-19 screening prior to your visit.

The day before your visit, you will receive a COVID-19 prescreening form via a secure link in the patient portal; please complete and submit. If you do not, you will have to complete the paper form which will delay your appointment. 

If you or a family member are experiencing COVID-19 symptoms, i.e., cough, fever, headache, loss of taste or smell, shortness of breath, in addition to contacting your primary care physician, please call our office to reschedule and discuss other options for services such as telehealth.
