  ARE YOU PROVIDING CARE TO A LOVED ONE AT HOME?
 
If so, you may qualify for Adult Foster Care. Adult Foster Care is funded by MassHealth and allows for a family member or friend to provide 24-hour care for a qualified member in their own home. The caregiver will receive a monthly, tax-free stipend for the care they provide.

   See If You Qualify: 

      

  • Must be 16 years of age or older

  • Must be unable to live alone because of a medical, physical, cognitive, or mental condition

  • Must need daily assistance with one or more activities of daily living (ADLs)

  • Cannot require full-time skilled nursing care

  • Must be willing to live with your caregiver

  • Must be eligible for MassHealth

  • Must be approved for AFC by a physician  

  • A nursing assessment determines the person’s level of need. 

  • The client can move in with the caregiver, or the caregiver can move in with the client

   Caregiver Requirements:

 

  • Must be 18 or older  

  • Caregiver cannot be spouse or another relative legally responsible for the care recipient(including parents of a minor child)

  • Caregiver cannot be the legal guardian

  • Maintain a private home in good repair and of sufficient size to accommodate both the AFC participant(s) and other residents of the home

  • Complete a physical exam including a TB test

  • Be physically able to supervise and/or provide hands on care to the client and assist with meal prep, transportation to doctor appointments, provide Rx monitoring and personal care

  • Respect participant's right to make decisions 

  • Pass a CORI check
